PGY2 Critical Care Application Information
Application Requirements
In order to be considered for this residency, the applicant must:- Possess a strong interest in critical care medicine clinical pharmacy practice
- Be eligible for employment in the United States and at University Health per Human Resources Department
- Complete an Accreditation Council for Pharmacy Education (ACPE) accredited PharmD degree program
- Be enrolled in or complete an ASHP-accredited PGY1 Pharmacy Residency
- Participate in the ASHP Resident Matching Program
Application Materials
The applicant must submit the following application materials via the Pharmacy Online Residency Centralized Application Service (PhORCAS) at https://www.ashp.org/phorcas- Letter of interest/intent
- Current curriculum vitae
- Three professional letters of recommendation
- Official college of pharmacy transcripts
