UHS to Implement PCCI's Pieces™ All-Cause Readmission Reduction Model
PCCI signs a 3-year contract with University Health System in San Antonio, Texas to implement the Pieces™ software platform.
"University Health System is a nationally recognized, premier teaching hospital system, and we are thrilled to partner with them around the Pieces™ system," states Ruben Amarasingham, MD, MBA president and chief executive officer of PCCI.
Within 24 hours of patients checking into the hospital, Pieces™ tells care providers who is most at risk for readmission. The technology takes a holistic view of the patient, analyzing clinical and social data found in the electronic medical record. Pieces™ then helps providers focus specific services and care on high-risk patients to guide them through their hospital stay and beyond discharge to keep them healthy and out of the hospital.
With the help of the Pieces™ technology, physicians, nurses, and case managers have early access to information that allows them to make better decisions about who needs the most intensive care. PCCI and University Health System will work together to optimize University Health System's existing care plans for high risk patients in order to get the best possible results from Pieces™ for patients at University Health System.
